Best Shoes for Orthotics: What to Look For (and What to Avoid)

Custom orthotics are only as effective as the shoes they're placed in. An excellent orthotic in the wrong shoe produces mediocre results — and sometimes makes things worse. The shoe's internal geometry, removability of its factory insole, and last shape all determine whether an orthotic can do its job correctly.

This is a frequently overlooked part of orthotic care. Most guides focus on what orthotics do; this one focuses on what shoes enable them to do it.


What Makes a Shoe Orthotic-Friendly

Removable Insole

The single most important feature. When a shoe's factory insole is removable, the custom orthotic replaces it, taking up only the space the factory insole previously occupied. The foot sits at the same height in the shoe as before — just supported differently.

When the factory insole is cemented in place, the orthotic must sit on top of it, raising the foot's position in the shoe by the full thickness of the orthotic. In most cases this causes heel slippage, toe crowding, and a generally wrong fit.

Look for: "removable insole," "extra depth," or "orthotic-ready" in product descriptions. Better: physically try to pull the insole out before buying.

Adequate Depth

A shoe with sufficient depth — height between the insole and the upper — accommodates the orthotic without pressing on the top of the foot. Shoes designed for orthotics often have extra depth (sometimes labeled "+1" depth or "extra depth") to account for insole thickness.

Without adequate depth, even a slim orthotic will create pressure on the dorsum of the foot, causing discomfort unrelated to the orthotic itself.

Lacing or Closure That Adjusts

Lace-up shoes allow adjustment around the orthotic. When an orthotic raises the foot position slightly or changes the foot's position in the shoe, laces can be loosened or retightened to accommodate. Slip-on styles — loafers, certain boots, mules — offer no adjustment.

Supportive Structure

A shoe with some structure and rigidity gives the orthotic a stable platform to work from. An orthotic in a flexible, unstructured shoe moves around and cannot maintain its posted position during dynamic loading.

Appropriate Last Width

The shoe's last should be wide enough to accommodate the orthotic without the sides of the orthotic compressing against the upper. Orthotics are made to the foot's width; if the shoe's last is significantly narrower, the orthotic will tilt or be forced into a distorted position.


Footwear Categories and Orthotic Compatibility

Running and Athletic Shoes

The most orthotic-compatible footwear category. Most running shoes have removable insoles, substantial depth, and lacing systems. The internal volume is designed to accommodate active feet, and the structure supports orthotics well.

What to look for in running shoes:

  • Removable sock liner
  • Neutral or stability category (motion control shoes already have heavy medial posting — can conflict with a custom orthotic's own posting)
  • Wide toe box to avoid forefoot crowding

Stability and motion control shoes should be used with care if you already have a custom orthotic — the shoe's own arch posting and the orthotic's posting can work at cross-purposes. For orthotic users, a neutral shoe + custom orthotic is typically the better combination than a stability shoe + orthotic.

Walking and Everyday Shoes

Walking shoes from brands that specifically design for orthotic accommodation — Ecco, Clarks, New Balance, Vionic, Dansko, Hoka (some models) — are generally excellent. Many are made with extra depth and removable insoles as a product feature.

New Balance in particular makes multiple-width running and walking shoes (2E, 4E) that provide more internal volume for orthotics than standard-width models.

Work and Safety Footwear

Work boots and safety shoes are often the most challenging after dress shoes. Many have cemented insoles, and the internal geometry is designed for a thick sock rather than an orthotic. Steel-toed boots have particularly constrained toe boxes.

Better choices for work orthotic users:

  • Brands that offer wide-width or extra-depth safety boots
  • Work shoes (lower profile) over full boots when the job permits
  • Checking that a thin work-specific orthotic fits correctly before committing to a full pair

Dress Shoes

Oxford-style lace-ups accommodate better than slip-ons; brands with orthotic-friendly dress shoe lines (Ecco, Mephisto, Clarks) are significantly better than traditional narrow-last dress shoes.

Sandals and Open Footwear

Sandals designed for orthotic use exist — primarily from Birkenstock (with removable footbeds), Vionic, and a few other brands. Standard sandals typically have no insole to remove and minimal structure, making orthotics difficult to retain and position correctly.

For patients who must wear sandals, a sandal with a deep footbed and adjustable straps (to accommodate the orthotic's height) is the best option. The orthotic needs to be retained under the foot, not sliding around in an open sandal.


Red Flags: Shoes That Don't Work With Orthotics

Cemented or non-removable insoles: The most common compatibility failure. The orthotic sits on top, raising the foot too high.

Pointed toe boxes: The narrowed forefoot doesn't accommodate a full-length orthotic's metatarsal area.

Ballet flats: Thin profile, no structure, often no insole to remove. Almost impossible to accommodate orthotics in most cases.

Very flexible or minimalist shoes: Without structure, the shoe can't provide the stable base the orthotic needs. The orthotic moves, tilts, and eventually becomes ineffective.

Narrow lasts in general: The orthotic was made to your foot's width. If the shoe is significantly narrower, the orthotic won't sit flat.

High heels over 2 inches: The biomechanics of a steep heel elevation undermine what most orthotics are trying to accomplish. And there's typically no depth for an insole.


How to Test a Shoe's Orthotic Compatibility Before Buying

  1. Remove the factory insole. If it doesn't come out, the shoe is not orthotic-friendly.
  2. Place your orthotic where the factory insole was. Does it lay flat? Does it fit the shoe's length and width without bunching?
  3. Put the shoe on with the orthotic in place. Walk around. Is there pressure on the top of the foot? Is the heel secure?
  4. If you have wide or narrow feet, or a thick orthotic, try the shoe in a different width before assuming the model doesn't work.

Do I need special shoes for custom orthotics?

My orthotics fit in one shoe but feel wrong in another. What's happening?
